Unusual case of massive gastrointestinal hemorrhage.

چکیده
A 49-year-old woman presented to the emergency department with bright red rectal bleeding, a hemoglobin level of 8.6 g/dL, and a hematocrit level of 27.8%. Because of complications of type 1 diabetes mellitus, the patient received a pancreaticoduodenal transplant with ileum drainage 8 years before presentation. This allograft failed 5 months before presentation, presumably owing to rejection. Bleeding necessitated nearly continuous high-volume fluid resuscitation. Results of angiography revealed perfusion of the pancreatic allograft (panel A, arrow) and extravasation of contrast material into the small bowel (panel B, arrow; imaged obtained a few seconds later). Interventional radiologic and vascular surgery services were not available. Despite heroic measures that included Fogarty catheter placement in the allograft arterial pedicle, the patient died of exsanguination. In a 14-hour period, she received 24 U of packed red blood cells, 22 U of fresh frozen plasma, other blood products, and crystalloids. The present case illustrates the potentially devastating complication of an arterioenteric fistula after an enteric-drained pancreaticoduodenal transplant, which has also been noted in other cases.1-4

Episodes of Massive Upper Gastrointestinal Bleeding Due to Dieulafoy’s Lesion in A 75-Year-Old Man: A Case Report
Aneurysms of gastric vessels (Dieulafoy`s disease, caliber-persistent artery) are thought to be of malformative rather than degenerative origin. They are usually single, located in the submucosa, usually on the lesser curvature, and characterized by a large tortuous vessel surmounted by a small defect...
متن کاملMASSIVE GASTROINTESTINAL HEMORRHAGE IN COMBAT CASUALTIES AND TRAUMA VICTIMS SECONDARY TO ARTERIO-VISCERAL FISTULAS
Massive upper gastrointestinal bleeding is a known complication of advanced sepsis in trauma patients and carries a grave prognosis. However, to our knowledge, an important cause of massive gastrointestinal bleeding, arterio-visceral fistula, has not been discussed in the literature. The most common cause of this unique clinical entity is penetrating high velocity abdominal injuries, altho...
متن کاملTraumatic Pseudoaneurysm of Superior Rectal Artery - an Unusual Cause of Massive Lower Gastrointestinal Bleed: A Case Report
Traumatic pseudoaneurysm of superior rectal artery is an unusual cause of massive lower gastrointestinal bleed. We are reporting the first case as we could not come across any similar report in the literature. Patient underwent exploratory laparotomy, diversion sigmoid loop colostomy, perineal wound debridement and antiseptic dressing for traumatic perineal wound. Patient had repeated episode o...
متن کاملMassive Obscure Overt Gastrointestinal Bleed: An Unusual Cause diagnosed by Capsule Endoscopy
Common causes for obscure overt gastrointestinal bleeding originating from the small bowel include small bowel angioectasias, Crohn’s disease, ulcers and tumors. In tropical countries, intestinal ulcerations secondary to tuberculosis and parasitic infestations have also been described as causes of gastrointestinal bleeding. We present a very unusual case of massive obscure overt gastrointestina...

INTRODUCTION Juvenile polyposis syndrome(JPS) is a rare autosomal dominant inherited condition. Hamartomatous polyps can affect the entire gastrointestinal tract but usually predominate in the colon. In this case report we present an unusual case of JPS that presented with massive gastric polyposis requiring a total gastrectomy. CASE PRESENTATION A 51-year-old man presented with symptoms of g...
متن کاملRecurrent lower gastrointestinal bleeding due to angiodysplasia in the rectum: endoscopic treatment with heater probe
Lower gastrointestinal bleeding due to vascular abnormalities is commonly found in the elderly and on the right side of the colon. Such lesions are still difficult to diagnose and manage. We report a case with recurrent and massive lower gastrointestinal bleeding due to angiodysplasia at an unusual age and localization, which was diagnosed and treated endoscopically.
